What You'll Learn

Promoting healthy ageing and care for seniors in the community involves the delivery of health and social care across various disciplines and sectors. This module aims to equip participants with the knowledge, skills, attitudes and competencies for effective case management. Specifically, participants will be taught on how to coordinate care across the care continuum and sectors with the aim of assisting clients and family members to navigate the complex healthcare system. Participants will also be taught on skills to leverage on community partners & resources for follow-up and support e.g. finance, transport, meals, escort services, medical equipment, appointments etc. Participants will be taught on the importance of maintaining effective communication and interpersonal relations.

Entry Requirements

a. Local Polytechnic Diploma or higher qualifications of any discipline which offer module(s) in any of the following areas:

• Allied Health
• Humanities and Social Sciences
• Sports Sciences
• Health Promotion
• Sports Coaching
• Public Health

b. Applicants with Local Polytechnic Diploma or higher qualifications but not taken any of the abovementioned academic modules e.g. Diploma in IT, may qualify for the programme with at least 2 years relevant working experience

c. Applicants do not meet the above entry requirement may be required to be further assessed through an interview and/or submission of a written essay on a given topic. Relevant experience encompasses additional related trainings, short course, workshops, or certifications obtained.
