Assessment and Management of Chronic Diseases
About This Course
Students will also gain a deeper understanding on how these chronic conditions impact the lives of the elderly living in the community. Importantly, students will be taught on identification of “red flags” and how to prevent and manage them.
What You'll Learn
This module is designed as a holistic module that will commence with preparation of students to be able to conduct an assessment of vital parameters related to selected common chronic medical conditions. Students will then follow through to understand the pathophysiology, etiology, predisposing factors, common diagnostic tests, nursing and medical management of the selected chronic conditions.
Entry Requirements
O Levels
• 3 relevant passes; and
• At least 1 year relevant work experience
Higher Nitec
• GPA ≥ 2.0; or
• GPA ≥ 1.5 and at least 1 year relevant work experience
Nitec
• GPA ≥ 3.5; or
•GPA ≥ 3.0 and at least 1 year relevant work
Higher Nitec in Technology / Services
• GPA ≥ 2.0 and at least 1 year relevant work experience
Nitec in Technology / Services
• GPA ≥ 3.5 and at least 1 year relevant work experience
WSQ
• Relevant WSQ Advanced Certificate (or higher); and
• At least 1 year relevant work experience; and
• Level 6 WSQ Workplace Literacy SOA and Workplace Numeracy SOA
Applicants who do not meet the entry requirements may be considered for admission to the course based on evidence of at least 2 years of relevant working experience. Suitable applicants who are shortlisted will have to go through an interview and/or entrance test. The Polytechnic reserves the right to shortlist and admit applicants.
