What You'll Learn
This Living Well course covers various aspects of effectively managing these diseases, including assessment and monitoring, medication, and lifestyle strategies. Learners will then be able to coach patients how to proactively manage their own conditions using existing tools to plan their meals and exercise routines. This includes teaching learners how to instil in patients the practice of reading nutritional labels in common food and drinks, enabling their patients to make informed dietary choices, take better control of their health and improve the quality of their lives. The enhancement of these skills is also aligned with the recent healthcare and population initiatives of "Healthier SG”, which focuses on community and lifestyle-based healthcare delivery, and National Plans for Successful Ageing. The skills learnt will allow participants to acquire the necessary skills sets in managing community and healthcare treatments in specialist and community clinics, and in senior activity centres.
