Care Coordination 101
About This Course
Course Objective
The objective of this course is to equip learners with the skills and knowledge about the fundamentals of Care Planning & Case Management and its significance in the Care process, from Hospital to Home, and how it can facilitate better collaboration and information sharing between community and care partners, to provide better & more holistic care to meet the evolving and specific needs of the individual senior.
This course is administered by Hua Mei Training Academy, an initiative of Tsao Foundation.
What You'll Learn
There are three (3) learning units (LUs) in this course.
LU1: Understand Care Planning, Case Management and the various Community resources available to support our ageing population.
LU2: Discuss common care transition issues and procedures from Hospital to Home (Community)
LU3: Examine how Care advocates play an integral role to bridge suitable services from the community to meet the needs of the seniors.
Entry Requirements
Those who are 21 years old and above with at least 3 years of working experience in any industry. The required skills and knowledge for this course are as follows:
● Education:
● be able to listen and speak English at a proficiency level equivalent to the Employability Skills System (ESS) Literacy Level 4
● be able to read and write English at a proficiency level equivalent to the Employability Skills System (ESS) Literacy Level 4
● be able to manipulate numbers at a proficiency level equivalent to Employability Skills System (ESS) Numeracy Level 4
● have basic languages / dialects relevant to the target client group.
● Have prior working experience with older adults will be an added advantage
● Be motivated to learn and have positive attitude towards work and commitments.
● Have interest in community and social work.
