Foundation Knowledge for Effective Care Delivery HC
About This Course
Course Objective
The objective of this course is to equip learners with the skills and knowledge about the fundamentals of Effective Care Delivery to the older person and supporting them to identify their needs through understanding of the ageing process, its trajectory, different aspects of ageing and its implication in care.
This course is administered by Hua Mei Training Academy, an initiative of Tsao Foundation.
What You'll Learn
There are four (4) learning units (LUs) in this course.
LU1: Understand the community care sector, ageing trajectory (Normal vs Abnormal Ageing), mental health and ageing of the older population.
LU2: Recognise the different profiles of older persons and their needs through Clinical Frailty Scale (CFS); their individual and cultural diversity; and the Purpose of Care (PCC).
LU3: Discuss various healthcare practices including quality improvement, workplace safety and health; Professional, legal and ethical healthcare practice.
LU4: Discuss the roles and responsibilities of the community healthcare workers in various community care Setting.
Entry Requirements
Those who are 21 years old and above with at least 3 years of working experience in any industry. The required skills and knowledge for this course are as follows:
● Education:
● be able to listen and speak English at a proficiency level equivalent to the Employability Skills System (ESS) Literacy Level 4
● be able to read and write English at a proficiency level equivalent to the Employability Skills System (ESS) Literacy Level 4
● be able to manipulate numbers at a proficiency level equivalent to Employability Skills System (ESS) Numeracy Level 4
● have basic languages / dialects relevant to the target client group.
● Have prior working experience with older adults will be an added advantage
● Be motivated to learn and have positive attitude towards work and commitments.
● Have interest in community and social work.
