About This Course
At the end of the programme, participants are expected to achieve the following objectives:
- To equip and strengthen social workers’ knowledge on dementia trajectory and care transition;
- To develop a deeper understanding on impact of dementia towards caregivers;
- To be able to work through the grief and loss experienced by persons with dementia and their caregivers;
- To enable participants to apply person centered care principles in working with persons with dementia and their caregivers;
- To acquire knowledge and the use of Advanced Care Planning (ACP) and Lasting Power of Attorney.
What You'll Learn
This course aims to provide an exchange and learning platform for social workers to discuss the management of persons with dementia and the care issues that their caregivers face. Through case discussion and experiential learning, participants will learn to integrate knowledge on dementia, the philosophy and principles of care, coping with the trajectory of illness till end of life, and translate knowledge and skills into practice.
