(SCTP Module) Integrated Community Care Practice and Reflection
About This Course
This module provides learners with the opportunity to apply their knowledge and skills in real community care workplace environments. Learners will undertake a supervised workplace-based learning attachment with approved community care partners, where they gain practical, hands-on experience in delivering person-centred, holistic care to community. Under the guidance, learners will perform care tasks, participate in interprofessional collaboration, and engage in reflective practice to deepen their understanding of population-focused health needs. Learners who are unable to undertake the workplace-based internship will complete a capstone project, allowing them to demonstrate their competencies by addressing authentic community care challenges. This applied learning approach ensures learners graduate with both practical exposure and a portfolio of evidence, enhancing their readiness to work effectively in community and healthcare settings.
