Dementia Care: Applied Case and Care Planning in Community Settings
About This Course
This course is an intensive, professional-grade training course designed strictly for professional care personnel—including Social Workers, Medical Social Workers, Social Work Associates, case coordinators, professional agency caregivers, and nursing aides. This course is explicitly not intended for informal or family caregivers who do not have professional care documentation or coordination responsibilities. The program equips professional learners with the practical competencies to apply person-centred care models (such as Tom Kitwood's psychosocial framework) to draft, document, monitor, and adapt comprehensive dementia care plans. Learners master skills to integrate safe communication keywords, behavioural redirection strategies, and structured cognitive engagement schedules into formal case files in alignment with organizational standards.
This course directly addresses Singapore's critical eldercare manpower demands and surging workforce needs caused by a rapidly aging population and the resulting rise in individuals living with cognitive decline. Frontline professionals often face performance gaps in systematic care tracking, environmental trigger identification, and dynamic care plan adjustments for behavioural symptoms like sundowning or agitation. Targeting competencies in Care Planning and Implementation, Case Management, and Dementia Care Support—this curriculum supports the Ministry of Health (MOH) and Agency for Integrated Care (AIC) "aging-in-place" masterplans. By upskilling practitioners with robust documentation and care orchestration capabilities, the program enables community care networks to deliver safe, effective, and sustainable dementia care within local neighbourhoods.
