About This Course
Participants will learn how to:
a. Engage effectively with older adults and stakeholders to identify changing needs.
b. Identify trends and societal factors relating to managing an ageing population.
c. Provide solutions to promote optimal ageing.
What You'll Learn
This module introduces coaching and case management frameworks and tools that will prepare participants to assist in developing customized care plans and activating suitable services and resources to meet client and family’s needs. Participants will also be equipped with knowledge and skills to educate clients on health management and available programmes and services so as to optimise support and empowerment.
Entry Requirements
At least a Polytechnic Diploma, or ITE Technical Diploma/Technical Engineer Diploma/Work-Learn Technical Diploma.
Applicants who do not meet the entry requirements may be considered for admission to the course based on evidence of at least 5 years of relevant working experience or supporting evidence of competency readiness. Suitable applicants who are shortlisted may have to go through an interview and/or entrance test. The Polytechnic reserves the right to shortlist and admit applicants.
