Case Management
About This Course
Students will gain an appreciation of the complexities in the coordination of care amongst various stakeholders such as the service providers, clients and their caregivers, while maintaining the desired health outcome and cost.
What You'll Learn
This module introduces students to the concept of case management within Singapore; its collaborative process in assessing and identifying needs of clients and their caregivers, developing care plans, and the coordination and mobilisation of resources available in the community.
Entry Requirements
O Levels
• 3 relevant passes; and
• At least 1 year relevant work experience
Higher Nitec
• GPA ≥ 2.0; or
• GPA ≥ 1.5 and at least 1 year relevant work experience
Nitec
• GPA ≥ 3.5; or
•GPA ≥ 3.0 and at least 1 year relevant work
Higher Nitec in Technology / Services
• GPA ≥ 2.0 and at least 1 year relevant work experience
Nitec in Technology / Services
• GPA ≥ 3.5 and at least 1 year relevant work experience
WSQ
• Relevant WSQ Advanced Certificate (or higher); and
• At least 1 year relevant work experience; and
• Level 6 WSQ Workplace Literacy SOA and Workplace Numeracy SOA
Applicants who do not meet the entry requirements may be considered for admission to the course based on evidence of at least 2 years of relevant working experience. Suitable applicants who are shortlisted will have to go through an interview and/or entrance test. The Polytechnic reserves the right to shortlist and admit applicants.
