About This Course
This course offers an extensive training program to equip volunteers with the knowledge, skills, and tools to engage and deliver care and support to community residents with long-term chronic diseases. The theoretical notions and ethical considerations of geriatric case management, root cause analysis, and sense-making are just a few of the subjects covered. Participants will learn best practices standards for engaging with individuals and families, addressing complex needs and challenges, and building strong partnerships with other service providers. By the end of the course, participants will have gained a thorough understanding of the fundamentals of community care. They will be better equipped to support individuals in need within their communities.
