About This Course
This course is tailored to healthcare professionals, particularly those in the community care sector, equipping them with the critical skills needed to coordinate and deliver a wide range of services that support seniors in home-based and community care settings. What You'll Learn
2. Apply ethical and legal principles in the handling and communication of sensitive patient information, adhering to best practices and ensuring compliance with relevant standard operating procedures (SOPs) and organizational guidelines.
3. Utilize effective handover and hand-off communication techniques to ensure the timely escalation of adverse changes in patient conditions.
4. Apply infection control practices by correctly using Personal Protective Equipment (PPE), adhering to both standard and transmission-based precautions, and identifying abnormal skin and respiratory conditions to minimize the risk of infection transmission.
5. Demonstrate proficient infection control management by performing proper sterilization and disinfection procedures, educating patients and caregivers on infection prevention, conducting regular infection control audits, and accurately reporting outcomes from contact tracing and audits.
6. Assess the client's needs and requirements by implementing an individualized care plan (ICP), considering the client's complex conditions, specific requirements, and factors that may influence their participation in care activities
7. Support group therapy planning and facilitation, contributing to the delivery of group therapy interventions for routine cases, while seeking guidance and collaborating with more experienced team members in the management of complex cases.
8. Assist in performing comprehensive health assessments and develop evidence-based care plans that address nursing diagnoses, ensuring holistic and patient-centered care in line with clinical best practices and pathways.
9. Implement nursing interventions for prescribed investigations, treatments, and discharge plan
Entry Requirements
• Obtained at least a C6 pass at N-Level in any 1 subject or equivalent; OR
• Graduate of HMI Institute’s WSQ HIGHER CERT IN HEALTHCARE / WSQ HIGHER CERT IN HEALTHCARE SUPPORT (any tracks/specialisations)
