What You'll Learn
At the completion of the PDC, learners will be able to:
(1) Explain the management for clients with chronic health conditions in intermediate and long-term care setting with reference to appropriate care transition concepts and framework;
(2) Apply evidence in neuroscience nursing in the development of neuroscience nursing practices using principles of evidence-based practice and appropriate appraisal framework; and
(3) Plan of care for client with neuromedical disorders using a comprehensive collaborative care approach.

Entry Requirements
• Diploma in Nursing from a local Polytechnic or an equivalent qualification;
• Singapore Nursing Board registration;
• 2 years post registration experience with a minimum of 1 year in the discipline of study.
